Making a Complaint
If you are unhappy with the service you receive from us, it is important that you let us know. We will investigate and try to resolve any issues locally but, if you are not happy with the way that we deal with your complaint, we will let you know how to refer the matter to a higher authority.
In the first instance, please address any complaints to Mr Jerry Nelson, our Practice Business Manager, but by sending to - email@example.com.
You may wish to speak to an independent advocate by calling Healthwatch's West Sussex Independent Health Complaints Advisory Service by ringing 0300 012 0122 or by emailing firstname.lastname@example.org.
