Avisford Medical Group welcomes new patients who live within our catchment area.

Our catchment area includes : Aldingbourne, Barnham, Bilsham, Binsted, (some of) Bognor Regis, Boxgrove, Climping, Eastergate, Elmer, Felpham, Fontwell, Ford, Middleton, Oving, Slindon, Woodgate and Yapton.

If you are unsure whether or not you live within our catchment area, please ask our reception staff.

From January 2015, the NHS made provision for GP practices to register patients who live outside their practice area - but entirely at their own discretion. Avisford Medical Group has a policy NOT to allow any out-of-area patients to register or to remain as existing patients if their home address is outside our catchment area.

The registration of a new patient involves the completion of some forms.

These forms can be accessed here or can be requested via Reception at either Yapton Surgery or Middleton Surgery.

Registering On-Line

Please use the following link to take you to SystmOnline - where you will find a form which you will need to complete in full - 

Once you have provided the necessary information needed to register you for NHS services at either of our two surgeries, you will also need to complete an Avisford Patient questionnaire (see below).

When we have all of the information that we need, we will arrange for the transfer of your medical record from your previous GP.

Please note that registering on-line involves the transmission of personal data via the internet - the absolute security of which we cannot guarantee.
