Physiotherapy self-referral

GP surgeries across West Sussex have now introduced online self-referral forms. This can be accessed via the website below (we can give you a form if you have no access at all to the website)

The physiotherapy department can no longer accept self-referrals by phone – all patients must complete the form below and submit it by email, post or by hand at GP Surgery. 

GP appointment for referral to see a Physiotherapist for muscular or joint related problems is not necessary if using self-referral form.
