Palliative Care
The Gold Standards Framework (GSF) is a practice-based system to improve the organisation and quality of palliative care services for patients who are at home in their last year of life. The practice regularly meets with a Specialist Nurse from St Wilfrid's Hospice and the Community Nursing Team.
